Kindergarten Enrollment:
1) Copy of Birth Certificate
2) Physician's Physical Report
3) Dental Report
4) Current Immunization Record
Transfer Student Enrollment:
1) Current Immunization Record
2) Copy of latest SAT/STAR testing or most recent report card
